Eye Examination Sample Clauses
Eye Examination. All employees who are Health Service System members shall be eligible for one (1) annual eye examination and prescribed eyewear for computer use.
Eye Examination. Eye examinations not covered by Alberta Health Care will be covered to a maximum of $55.00 per examination and shall be limited to one examination per covered person per year.
Eye Examination. One examination for each covered person within any 12 month period.
Eye Examination. (a) When a majority of an employee's daily work time requires monitoring such VDT's, such employees shall have their eyes examined by an ophthalmologist of the employee's choice at the nearest community where medical facilities are available, and after six (6) months a further test, and annually thereafter, if the employee so requests. The examination shall be at the employer's expense where costs are not covered by insurance. Time off work to attend the eye test shall be granted with pay.
(b) Where it is determined that glasses are required, the Board shall pay up to a maximum of $100.00 towards the cost of those glasses which is in excess of the coverage in the EHB Plan.
Eye Examination. Parents/guardians are encouraged to have their children undergo an eye examination whenever health examinations are required. Parents/guardians of students entering kindergarten or an Illinois school for the first time shall present proof before October 15 of the current school year that the student received an eye examination within one year prior to entry of kindergarten or the school. A physician licensed to practice medicine in all of its branches, or a licensed optometrist, must perform the required eye examination. If a student fails to present proof by October 15, the school may hold the student’s report card until the student presents proof: (1) of a completed eye examination, or (2) that an eye examination will take place within 60 days after October 15. The Director or designee shall ensure that parents/guardians are notified of this eye examination requirement in compliance with the rules of the IDPH. Schools shall not exclude a student from attending school due to failure to obtain an eye examination.
Eye Examination. To provide an eye examination up to $90 per family member per 24 months in each year of the Agreement. • Where an employee is required to have additional eye examinations due to other medical reasons, the Company will pay the cost up to a total of $500.00 for every twenty-four (24} months.
Eye Examination. 10/5/1 If an eye examination for safety glasses is necessary, the Employer will pay the entire cost of one examination during the life of this Agreement.
Eye Examination. Effective June 1, 2006, the Employer shall pay the full cost of one (1) eye examination every twenty-four months (24) in addition to the aforementioned maximum amounts for vision care.
Eye Examination. Full time and regular part time employees of the Department are eligible for reimbursement for eye examinations according to the following:
1. A full time employee may receive a maximum reimbursement in any calendar year for out of pocket expenses not to exceed $75.
2. A regular part time employee who has been continuously employed at NAED for 12 months or more may receive reimbursement for a percentage of his/her out of pocket costs for eye examinations (up to the maximum provided, below). The reimbursement percentage will be determined by dividing the number of hours for which the employee has been paid during the preceding 12 months by 2080 hours. The employee’s reimbursement will be the lesser of:
1.) the amount of the out of pocket costs for the examinations multiplied by the reimbursement percentage, or
2.) $75 multiplied by the reimbursement percentage.
3. A regular part time employee who has been continuously employed at NAED for less than 12 months may receive reimbursement for a percentage of his/her out of pocket costs for eye examinations (up to the maximum provided, below). The reimbursement percentage will be determined by dividing the number of hours for which the employee has been paid during the preceding 12 months by the number of weeks that the employee has been employed during the preceding 12 months and then dividing the resulting number by forty (40). The employee’s reimbursement will be the lesser of:
1.) the amount of the out of pocket costs for the examinations multiplied by the reimbursement percentage, or
2.) $75 multiplied by the reimbursement percentage.
Eye Examination. The Company agrees to pay the cost of eye exams to a maximum of $150.00 every 2 years.
