Common use of Extended Health Coverage Clause in Contracts

Extended Health Coverage. Deductible The deductible is twenty-five dollars ($25) each benefit year for each person up to a maximum of twenty- five dollars ($25) per family. Prescription Drugs The plan will cover the cost of drugs (lowest priced equivalent) which by law are only available with a prescription as long as they are prescribed by a doctor or dentist and are obtained from a pharmacist. Intrauterine devices (IUDs), diabetic and colostomy supplies are also covered. The plan will cover eighty percent (80%) of the above after deductible is paid.
