Exceptions Procedure. The employee or his/her supervisor will request permission of the district office Director of Human Resources shortly before or after the time needed where exceptions may apply. The request may be either verbal or written. The Director of Human Resource’s response will be in writing.
Exceptions Procedure. The plan includes an exceptions procedure that can be applied in situations in which a prescribing physician insists on non-formulary medications which are to be taken over a long period of time, with the result that an employee would be expected to incur large copayments. Under the exceptions procedure, it will be possible for non-formulary copayments (25% of cost, but not less than $5, and not more than $25) to be reduced to the levels associated with formulary (preferred) drugs (10% of cost, but not less than $5, and not more than $25). Under the exception’s procedure, an employee may request that a long-term course of therapy involving non-formulary drugs be referred to an independent clinician, either a pharmacist or a physician, for review.
