Evaluation Services. If We give You access to the Services for evaluation purposes, You can only use the Services in a non-production environment and for the period We notify You of in writing. If there is no evaluation period identified, then the Services will be available for 30 days from when You first access the Services. Regardless of any other terms in this Agreement, the Services provided for evaluation purposes are provided “AS-IS” without any express or implied support or warranty of any kind. If You violate this Section, We may charge You fees for Your use.
Evaluation Services. The Manager shall oversee and monitor the performance of the Portfolios' investment adviser and shall furnish to the Trust such information, evaluations, analyses and opinions regarding said performance as the Trustees may, from time to time, reasonably request; provided, however, that the Manager shall have no authority to make and shall not make investment decisions for the Portfolios nor furnish any advice with respect to the desirability of making such investment decisions.
Evaluation Services. Arista may make the Subscription Services available to Customer for evaluation, training, beta testing or for other non-commercial purposes (“Evaluation Services”). Customer may use the Evaluation Services without paying Arista, subject to the following conditions: (i) Customer may use the Evaluation Services for evaluation purposes only in a non-production environment; (ii) use of any Software provided to Customer, either embedded on hardware or separately, as part of the Evaluation Services is subject to Xxxxxx’x XXXX as an evaluation license; (iii) NOTWITHSTANDING ANY EXPRESS WARRANTIES OR INDEMENITIES PROVIDED HEREIN BY ARISTA CUSTOMER ACKNOWLEDGE AND AGREES THAT THE EVALUATION SERVICES ARE PROVIDED “AS IS” WITH NO WARRANTIES OF ANY KIND AND ARISTA HAS NO INDEMNIFICATION OBLIGATION WITH RESPECT TO THE EVALUATION SERVICES; and (iv) upon the termination or expiration of the Evaluation Services, Customer will return any hardware made available as part of the Evaluation Services to Arista, in a manner agreed to by the parties. Arista may revoke or suspend Customer’s access to the Evaluation Services at any time for any reason. Arista has no obligation to retain Customer data held by Arista in the course of providing the Evaluation Services
Evaluation Services. Anaplan may invite Client to use Evaluation Services at no charge and Client may accept or decline the invitation in its sole discretion. Evaluation Services are for evaluation and testing purposes, not for production use, not supported, not subject to availability or security obligations and may be subject to additional terms. Unless otherwise agreed by Anaplan in writing, the Evaluation Services trial period will expire upon the earlier of six (6) months after the date Anaplan first makes the Evaluation Services available to Client, the date Anaplan in its discretion makes a version of the Evaluation Services generally available, or earlier termination of this Agreement. Anaplan may discontinue Evaluation Services at any time in Anaplan’s sole discretion and may never make them generally available. Anaplan will have no liability for any harm or damage arising out of or in connection with Evaluation Services.
Evaluation Services. In consideration of the fees by CLIENT in Section 3 of this contract, CLIENT is submitting a wastewater treatment system described as , hereinafter called MODEL. CLIENT desires to have GCT evaluate MODEL in accordance with the NSF/ANSI Standard , hereinafter referred to as STANDARD. MODEL shall be operated in accordance with CLIENT’s operating instructions and the applicable provisions of STANDARD. CLIENT is responsible for assuring proper installation of MODEL at GCT’s testing facility in Prairieville, Louisiana and shall sign a form indicating MODEL was properly installed prior to the start of the evaluation. Fees and arrangements for installation and removal of the MODEL are the responsibility of the CLIENT. As part of the evaluation services, the CLIENT agrees to participate in and/or provide the following information:
Evaluation Services. Psychological Testing is a service provided for the purposes of diagnostic clarification and development of treatment planning. It is often a useful tool in the course of psychological intervention and/or educational planning. In addition to an interview where you (or you and your child) will be asked questions about your background and current concerns, and strengths, you will also be administered standardized tests, including but not limited to asking questions about your knowledge of certain topics, reading, drawing figures and shapes, viewing printed material, and manipulating objects. Each psychological assessment tool is specifically chosen to answer the referral question(s). The selection of assessment tools attempts to maximize the validity of the results, while minimizing time and cost. Tests must be properly administered, scored, interpreted, and then a comprehensive psychological or neuropsychological report is written. It can take about 3 to 4 weeks from the time the last test data is received for a written report to be completed. Individuals understandably enter the testing process with many different expectations; however, no diagnosis or outcome is ever guaranteed. Testing is often performed by a psychologist or psychology trainee under the supervision of a licensed psychologist. However, sometimes we use a psychometrist, who is trained to administer psychological and neuropsychological tests, but this is done under the direct supervision of the psychologist, and all final evaluation results are solely the professional renderings of the psychologist. In following test protocols, typically no one other than the examinee and examiner will be in the room during testing. We realize that younger children or those with developmental disabilities may be unaccustomed to a setting in which a caregiver is not present. In these scenarios, we will work with caregivers to determine the best plan. For instance, caregivers may be present at the start of the appointment and exit once the child has developed rapport with the examiner and one-on-one testing can begin. The one exception is ASD testing with toddlers, in which caregiver presence in the room is required for the assessment.
Evaluation Services. My fee for psychological and neurodevelopmental testing includes administering and scoring the tests, writing the report, and discussing the results with the client or the parents/guardians. Many insurance companies cover psychological testing. Almost all, however, require the clinician to request authorization for the number of units (hours) that will be used to administer, score, and interpret that testing as well as write the report. Many insurance companies routinely authorize far fewer units than the evaluation requires. If your insurance company requires that I conduct the evaluation for the number of units they approve and they approve less than what is required, I must either take short cuts with the evaluation -which I have always refused to do – or conduct the evaluation for a fee that is far below the acceptable rate for such and evaluation. For these reasons I agree to request authorization from your insurance company to conduct the evaluation but if they authorized fewer units that what is required and they prohibit “balance billing” (billing the family for the amount that they do not cover). I will refuse to accept insurance payment for psychological testing. I will inform you of this decision and you may either negotiate a few with me for the evaluation, appeal the decision by the insurance company ( I will charge a nominal fee for completing additional forms or engaging in phone calls with the insurance company), or you may seek a different provider.
