Common use of Doctor’s Note Clause in Contracts

Doctor’s Note. The Employer may request a doctor’s note or other documentation from the employee upon the employee’s return to work after three (3) consecutive days off sick, or upon returning to work after being off sick on the last scheduled day before or after a holiday, or on a holiday that the employee was scheduled to work.
