CSH Services. CSH Services shall be provided by UAF to the Cardholder on and subject to the following terms and conditions: (a) I may use the CSH Service by giving Telephone Instructions and authorize UAF to act on such Telephone Instructions. UAF may exercise its discretion to accept or reject such Telephone Instruction. UAF will not be liable to me for any loss or damage suffered by me resulting from UAF’s exercise of its discretion to act or refusal to act upon such Telephone Instructions. Accordingly: (i) any Telephone Instruction, once given, may not be rescinded or withdrawn without the written consent of UAF; (ii) all Telephone Instructions given, as understood and acted on by UAF in good faith, shall be irrevocable and binding on me whether given by me or by any other person purporting to be a Cardholder; and (iii) UAF shall be under no duty to verify the identity or authority of the person giving any Telephone Instruction or the authenticity of such Telephone Instruction. (b) UAF shall be entitled to prescribe, from time to time, the telephone number through which Telephone Instructions must be given. (c) Telephone Instructions, to be effective, must be given by using whichever PIN as may be required, and such other details as UAF may require and the aforementioned must be in a form that is acceptable to UAF.
CSH Services. CSH Services shall be provided by PrimeCredit to the Cardholder on and subject to the following terms and conditions: (a) I may use the CSH Services by giving Telephone Instructions and authorize PrimeCredit to act on such Telephone Instructions. PrimeCredit may exercise its discretion to accept or reject such Telephone Instruction. PrimeCredit will (i) any Telephone Instruction, once given, may not be rescinded or withdrawn without the written consent of PrimeCredit; (ii) all Telephone Instructions given, as understood and acted on by PrimeCredit in good faith, shall be irrevocable and binding on me whether given by me or by any other person purporting to be the Cardholder; and (iii) PrimeCredit shall be under no duty to verify the identity or authority of the person giving any Telephone Instruction or the authenticity of such Telephone Instruction. Accordingly: (i) any Mobile Instruction, once given, may not be rescinded or withdrawn without the written consent of PrimeCredit; (ii) all Mobile Instructions given, as understood and acted on by PrimeCredit in good faith, shall be irrevocable and binding on me whether given by me or by any other person purporting to be the Cardholder; and (iii) PrimeCredit shall be under no duty to verify the identity or authority of the person giving any Mobile Instruction or the authenticity of such Mobile Instruction. (b) PrimeCredit shall be entitled to prescribe, from time to time, the telephone number or other platforms (e.g. websites or mobile applications, etc.) through which Mobile Instructions must be given. (c) For Mobile Instructions to be effective, PrimeCredit will use the PIN or such other details as PrimeCredit may require to verify or authenticate my identity when I access or use the CSH Services.
CSH Services. CSH Services shall be provided by Entrust to the Cardholder on and subject to the following terms and conditions: 4.1. Cardholder may use the CSH Services by giving Telephone Instructions and authorize Entrust to act on such Telephone Instructions. Accordingly: a. any Telephone Instruction, once given, may not be rescinded or withdrawn without the written consent of Entrust; b. all Telephone Instructions given, as understood and acted on by Entrust in good faith, shall be irrevocable and binding on the cardholder whether given by the cardholder or by any other person purporting to be the Cardholder; and c. Entrust shall be under no duty to verify the identity or authority of the person giving any Telephone Instruction or the authenticity of such Telephone Instruction. 4.2. Entrust shall be entitled to prescribe, from time to time, the telephone number through which Telephone Instructions must be given. 4.3. Telephone Instructions, to be effective, must be given by using whichever PIN as may be required, and such other details as Entrust may require and must be accepted by Entrust by such means as Entrust may prescribe.

This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.

  • Pharmacy Services The Contractor agrees to comply with the requirements regarding covered pharmacy and over-the- counter (OTC) benefits. The Contractor will comply with the EOHHS Pharmacy Home Program and the Generics First Initiative, including the maintenance of the drug formulary in accordance with the direction of the EOHHS Pharmacy Committee.

  • OUR SERVICES As insurance intermediaries we generally act as the agent of our client. We are subject to the law of agency, which imposes various duties on us. However, in certain circumstances we may act for and owe duties of care to other parties, including the insurer. We will advise you when these circumstances occur, so you will be aware of any possible conflict of interest. We offer a wide range of products and services which may include: • Offering you a single or range of products from which to choose a product that suits your insurance needs; • Advising you on your insurance needs; • Arranging suitable insurance cover with insurers to meet your requirements; • Helping you with any subsequent changes to your insurance you have to make; • Providing all reasonable assistance with any claim you make. Where we act on behalf of the insurer and not you, we will notify you accordingly and in relation to claims we will advise you of this fact when you notify us of a claim. Depending on the type of cover you require and where we have provided advice based on a personal recommendation, we will offer you a policy from either: • a single insurer; • a limited range of insurers; or • a fair analysis that is representative of the insurance market. We will advise you separately as to which of these apply before we arrange your policy and where we have not undertaken a fair analysis of the market, we will provide you with a list of insurers considered. For all other policies, including optional additional products and premium finance (if relevant), before the insurance contract is concluded and after we have assessed your demands & needs, we will provide you with advice and make a personal recommendation. This will include sufficient information to enable you to make an informed decision about the policy that we have recommended, together with a quotation which will itemise any fees that are payable in addition to the premium.
