Common use of Assignment of Benefits and Authorization to Pursue Third Party Payment Clause in Contracts

Assignment of Benefits and Authorization to Pursue Third Party Payment. By execution of this Agreement, the Resident, Resident Representative and Sponsor agree to assign to the Facility any and all applicable insurance benefits and other third party payment sources to the extent required by the Facility to secure reimbursement for the care provided to the Resident and authorize the Facility to seek and obtain all information and documentation necessary for the processing of any third party claim.

Assignment of Benefits and Authorization to Pursue Third Party Payment. By execution of this Agreement, the Resident, Resident Designated Representative and and/or Sponsor agree to hereby assign to the Facility any and all applicable insurance benefits and other third third-party payment sources to the extent required by the Facility to secure reimbursement for the care provided to the Resident and Resident. The Resident, Designated Representative and/or Sponsor authorize the Facility to seek and obtain all information and documentation necessary for the processing of any third third- party claimclaim(s).

By execution of this Agreement, the Resident, Resident Resident’s Spouse, Financial Sponsor and Designated Representative jointly and Sponsor agree to assign to the Facility any and all applicable insurance benefits and other third party payment sources to the extent required by the Facility to secure reimbursement for the care provided to the Resident and authorize severally authorizes the Facility to seek and obtain all information and documentation necessary for the processing of any third party claim.
