Annual Wellness Screening. The District will provide an annual wellness event which includes bio-metric screening. Bio-metric screening completed at school or with their own doctor during the designated annual window, will also qualify. The completion of an appropriate annual bio-metric screening which occurs between January 1 and April 30 will qualify the employee for the insurance contribution levels listed in the applicable section of 11.06.12 or 11.06.13.

The completion of an appropriate annual bio-metric screening which occurs between January 1 and April 30 will shall qualify the employee for the insurance contribution levels rates listed in the applicable section of 11.06.12 or 11.06.1313.04.2.
