Annual Wellness Screening. The BOARD shall offer, at its own expense, an annual wellness screening for all full-time TEACHERS covered. Those TEACHERS who do not elect to obtain a wellness screening or do not otherwise provide evidence of obtaining a wellness screening from his/her own physician shall contribute an additional $20 per month toward the cost of insurance coverage. For the purpose of the screening, annual refers to the period of twelve (12) months beginning the first day of each school year. The HR Department will provide annual notification of wellness screening procedures. The wellness screening procedure and forms can be found on the District website within the staff password-protected portal.

Annual Wellness Screening. The district shall provide an annual wellness event, which includes a biometric screening and questionnaire. Biometric screenings completed by the employee’s doctor or through a reputable lab during the designated annual window, along with completion of the questionnaire, shall also qualify. Off-site screenings must be completed between January 1st and April 30th and will qualify the employee for the insurance rates listed above.
Annual Wellness Screening. The District will provide an annual wellness event which includes bio-metric screening. Bio-metric screening completed at school or with their own doctor during the designated annual window, will also qualify. The completion of an appropriate annual bio-metric screening which occurs between January 1 and April 30 will qualify the employee for the insurance contribution levels listed in the applicable section of 11.06.12 or 11.06.13.

The parties agree that the failure of VENDOR to perform any of the duties described in this section shall constitute a material breach of this Agreement entitling SBBC to terminate immediately with no further responsibilities or duties to perform under this Agreement. VENDOR agrees to indemnify and hold harmless SBBC, its officers and employees from any liability in the form of physical or mental injury, death or property damage resulting from VENDOR’s failure to comply with the requirements of this section or with Sections 1012.32 and 1012.465, Florida Statutes.
