Common use of Access to PHI Clause in Contracts

Access to PHI. To the extent applicable, Business Associate agrees to provide access, at the request of Covered Entity, and in the time and manner designated by Covered Entity, to PHI in a Designated Record Set, to Covered Entity or, as directed by Covered Entity, to an Individual in order to meet the requirements under 45 CFR § 164.524 (if Business Associate has PHI in a Designated Record Set).

Access to PHI. To the extent applicable, The Business Associate agrees shall provide an Individual access to provide access, PHI in a Designated Record Set as required by 45 Code of Federal Regulations section 164.524; and at the request of the Covered Entity, and in the time and manner designated by the Covered Entity, the Business Associate shall provide access to PHI in a Designated Record Set, Set to Covered Entity or, as directed by Covered Entity, to an Individual in order to meet the requirements under 45 CFR § 164.524 (if Business Associate has PHI in a Designated Record Set)Individual.

Access to PHI. To the extent applicable, Business Associate agrees to provide access, at the request of Covered Entity, and in the time and manner designated by Covered Entity, access to PHI in a Designated Record SetSet to the Covered Entity. If an Individual makes a request for access pursuant to 45 C.F.R. § 164.524 directly to Business Associate, or inquires about his or her right to access, Business Associate shall forward it to Covered Entity or, as directed by Entity. Any response to such request shall be the responsibility of Covered Entity, to an Individual in order to meet the requirements under 45 CFR § 164.524 (if Business Associate has PHI in a Designated Record Set).

Access to PHI. To the extent applicablethat Business Associate maintains PHI in a Designated Record Set, Business Associate agrees to shall provide access, at the request of Covered Entity, Entity and in the time and manner designated reasonably specified by Covered Entity, to the PHI in a such Designated Record Set, to Covered Entity or, as directed by Covered Entity, to an Individual in order to meet the access requirements under 45 CFR C.F.R. § 164.524 (if Business Associate has PHI in a Designated Record Set)164.524.

To the extent applicable, Business Associate agrees to shall provide access, at the request of Covered Entity, and in the time and manner reasonably designated by Covered Entity, to PHI Protected Health Information in a Designated Record Set, to Covered Entity or, as directed by Covered Entity, to an Individual in order to meet the requirements under the Privacy Rule at 45 CFR C.F.R. § 164.524 (if Business Associate has PHI in a Designated Record Set)164.524.

Access to PHI. To the extent applicable, Business Associate agrees to shall provide access, at the request of Covered Entity, and in the time and manner designated by Covered Entity, to PHI in a Designated Record Set, to Covered Entity or, as directed by Covered Entity, to an Individual in order individual to meet the requirements under 45 Title 45, Part 164, Subpart E, Section 164.524 of the CFR § 164.524 (if and applicable state law. Access will be provided at the request of Covered Entity and at a time and in a manner designated by Covered Entity, to an Individual in order to meet the requirements under 45 CFR § 164.524 (if Business Associate has PHI in a Designated Record Set).
