Virginia Uniform Assessment Instrument (UAI) definition

Virginia Uniform Assessment Instrument (UAI) or "UAI" means the standardized, multidimensional questionnaire used to assess that assesses an individual's social, physical and mental health, and social and functional abilities. Under these regulations, the UAI is used to gather the information needed to determine an individual's long-term care needs and PACE plan service eligibility, for planning the care to be provided, and for monitoring care as it is provided.
