Written Report. To provide a written report of the investigation to the COUNTY Compliance Officer within ten (10) working days of the discovery of the breach or unauthorized use or disclosure. The report shall include, but not be limited to, the information specified above, as well as a full, detailed corrective action plan, including information on measures that were taken to halt and/or contain the improper use or disclosure.
Written Report. The Participant shall provide a written report of the investigation to the CDPH Program Manager, the CDPH Privacy Officer, and the CDPH Chief Information Security Officer within five (5) working days of the discovery of the breach or security incident. The report shall include, but not be limited to, the information specified above, as well as a full, detailed corrective action plan, including information on measures that were taken to halt and/or contain the breach or security incident, and measures to be taken to prevent the recurrence of such breach or security incident.
Written Report. Within twenty-four (24) hours after the telephonic notice, the Contractor shall submit to the Authority a report that describes the injury, theft, fraud or damage, the amount of theft, fraud or damage as best can be determined, the action the Contractor has taken or intends to take, and in the case of theft, fraud or deliberate damage by an employee of the Contractor, the name and job title of the employee who committed the theft, fraud or damage, action taken in addition to the information required above, and any other information relevant to the interests of the Authority. For the purposes of this Contract, theft shall include, but not be limited to, fraudulently removing parking tickets from a ticket issuing machine, intentionally failing to turn in all tickets at the end of shift, filing a false lost ticket claim, taking of cash receipts, intentional mischarging of customers, taking a patron’s vehicle or any part of it, or taking any article left in or on a patron’s vehicle. If not all information is available at the time of the initial report, the Contractor shall update the report as necessary until it is complete.
Written Report. Each Unit Member returning from leave shall file a written report with the Superintendent not later than 60 days after the return to active duty. This report shall include the name of the institution attended, courses pursued, credits received, experience gained, or the itinerary of travel, together with the Unit Member's appraisal of the professional value of the year's activities and the manner in which the knowledge and experience gained may be applied to the benefit of the pupils in the District. The report shall specifically indicate the manner in which the Unit Member has met the objectives stated in the sabbatical leave request.
Written Report. Within forty-five (45) days after the end of each fiscal quarter of an Earn-Out Year (except for the fourth fiscal quarter of each fiscal year, for which the applicable time period shall be ninety (90) days after the end of such fiscal quarter), Buyer shall calculate the DSOI for such Earn-Out Year (as measured through such fiscal quarter) and shall deliver to Sellers a report setting forth in reasonable detail the amount and determination of DSOI for such Earn-Out Year (as measured through such fiscal quarter). Each written report delivered to Sellers shall be accompanied by the quarterly financial statements used to calculate DSOI. Each written report and the financial statements accompanying such report for any fiscal quarter are collectively referred to herein as the “Written Report.” The Written Report shall be used for purposes of determining the Earn-Out Payment, if any, to be delivered to Sellers for such fiscal quarter in accordance with Section 2.3(c).
Written Report. Faculty Member must submit a written report in accordance with the process and format established by the institution outlining his/her accomplishments during the leave and include an accounting of all compensation received while on sabbatical in accordance with #3. above, on the attached form within three months of returning to Institution. These reports are to be filed and maintained in the Vice Chancellor’s Office and be available upon request.
