Common use of Unsecured Protected Health Information Clause in Contracts

Unsecured Protected Health Information. “Unsecured Protected Health Information” or “Unsecured PHI” shall mean PHI that is not secured through the use of a technology or methodology specified by the Secretary in guidance or as otherwise defined in the §13402(h) of the HITECH Act.

Unsecured Protected Health Information. “Unsecured Protected Health Information” or “Unsecured PHI” shall mean means PHI that is not secured through the use of secured: (1) by a technology or methodology specified by the Secretary in guidance or as otherwise defined in the §13402(h) of the HITECH ActDepartment of Health and Human Services (“HHS”); or (2) by a technology standard that renders PHI at rest or PHI in motion unusable, unreadable, or indecipherable to unauthorized individuals and that is developed or endorsed by a standards developing organization accredited by the American National Standards Institute.

Unsecured Protected Health Information. “Unsecured Protected Health Information” or “Unsecured PHI” shall mean PHI that is not secured rendered unusable, unreadable, or indecipherable to unauthorized individuals through the use of a technology or methodology specified by the Secretary in guidance or guidance, as otherwise defined in the §13402(h) of the HITECH Actset forth as 45 C.F.R. § 164.402.

Unsecured Protected Health Information. “Unsecured Protected Health Information” or “Unsecured PHI” " shall mean PHI Protected Health Information (PHI) that is not secured through the use of a technology or methodology specified by the Secretary in guidance regulations or as otherwise defined in the §13402(h) of the HITECH Act.

Unsecured Protected Health Information. “Unsecured Protected Health Information” or “Unsecured PHI” shall mean PHI that is not secured through the use of a technology or methodology specified by the Secretary in guidance or as otherwise defined in the §§ 13402(h) of the HITECH Act.

Unsecured Protected Health Information. “Unsecured Protected Health Information” or “Unsecured PHI” shall mean PHI Protected Health Information (PHI) that is not secured through the use of a technology or methodology specified by the Secretary in guidance regulations or as otherwise defined in the §13402(h) of the HITECH Act, Section 13402(h)(1).

Unsecured Protected Health Information. “Unsecured Protected Health Information” or “" and/or "Unsecured PHI” shall mean PHI " refer to information that is not secured through the use of a technology or methodology specified identified by the Secretary in guidance or as otherwise defined in the §13402(h) of the HITECH Actto render PHI unusable, unreadable and undecipherable to unauthorized users.

Unsecured Protected Health Information. “or Unsecured Protected Health Information” or “Unsecured PHI” PHI shall mean PHI that is not secured through the use of a technology or methodology specified by the Secretary of DHHS in guidance or as otherwise defined in the §13402(h) of the HITECH Act45 C.F.R. Section 164.402.

Unsecured Protected Health Information. “Unsecured Protected Health Information” or and/or “Unsecured PHI” shall mean PHI means information that is not secured through the use of a technology or methodology specified identified by the Secretary in guidance or as otherwise defined in to render the §13402(h) of the HITECH ActProtected Health Information unusable, unreadable and undecipherable to unauthorized users.

Unsecured Protected Health Information. “Unsecured Protected Health Information” or “Unsecured PHI” shall mean PHI that is not secured through the use of a technology or methodology specified specified by the Secretary in guidance or as otherwise defined defined in the §13402(h) of the HITECH Act. Terms used, but not otherwise defined, in this BAA (or the Services Agreement) shall have the same meaning as those terms in 45 C.F.R. §§ 160.103 and 164.501.

Unsecured Protected Health Information. “Unsecured Protected Health Information” or “Unsecured PHI” shall mean PHI that is not secured through the use of a technology or methodology specified by (such as encryption) that the Secretary specifies in guidance renders PHI unusable, unreadable, or indecipherable to unauthorized individuals, such as otherwise defined the guidance set forth in the §13402(h) of the HITECH Act74 Fed.
