Time off Request Clause Samples

A Time Off Request clause outlines the procedures and requirements for employees to formally request time away from work. Typically, it specifies how much advance notice must be given, the method for submitting requests (such as through an online system or written form), and any limitations on when or how much time off can be taken. This clause ensures that both employees and employers have clear expectations regarding time off, helping to maintain adequate staffing and minimize workplace disruptions.
Time off Request. If an employee, within a six (6) month period, requests time off whether it is holiday time, compensatory time, or vacation, the Department has an obligation to make a good faith response to the employee’s time off request within one (1) week of when the request is made. The employer will not automatically deny these requests in an effort to avoid responding to them in a timely fashion.
Time off Request. All requests for time off must be submitted through mutually agreed upon shift scheduling software/program. This shall be the master schedule of time off. For purposes of this section, time off shall be any paid leave except sick leave. Options for selection of time off shall be as follows:
Time off Request. 65.01 Time off requests must be made through the Employee Self Service Kiosk (ESS) twenty-four (24) hours prior to the applicable schedule being posted. The time off request will be approved or denied and displayed on the Employee Self Service Kiosk (ESS) within five (5) days of the original submission of the request. With the approval of the Manager, Supervisor or departmental designate, the time period may be abridged to forty-eight (48) hours provided the days off requested are available.
Time off Request. All requests for time off must be in writing and must be approved by the highest ranking company officer on duty. The calendar in the day room at station 2 shall be the master schedule of time approved. For purposes of this section, time off shall be any paid leave except sick leave. Options for selection of time off shall be as follows:
Time off Request. The employee is responsible for knowing the balance of their various time-off accounts, which may be obtained via their payroll check stub. If an employee is unsure as to the balance of their various time-off accounts, they should contact the Department Director directly. If the Department Director is unavailable, the employee shall leave a slip asking for their balance. All time-off requests must be submitted in writing, using the Benefit Time Request section of the Village’s Benefit Time Request /Absence from Work Report Form and submitted to the employee’s supervisor. For all time off from work, upon return to work, the employee must fill out the Absence from Work Report section of the Village’s Benefit Time Request/Absence from Work Report Form, and give it to the employee’s supervisor.
