Submission of Annual Representations and Certifications. The CNA shall review the Representations & Certifications submitted by NPAs to identify any errors, omissions, or anomalies as compared to prior submissions, regulatory review and assistance visit reports, and guidance from the Commission; and attempt to resolve them with the NPA within the first thirty (30) days of submission. The CNA shall, as part of the terms of the written agreement with the NPAs, include a provision in which the NPAs agree to provide accurate, timely and ongoing submissions for the Representations and Certifications form. The CNA shall forward to the Commission, by December 1 annually: (1) a completed original and legible copy of the annual Representations & Certifications signed by the NPAs, and (2) Data Extract of Annual Representations & Certifications containing all information within the annual Representations & Certifications and a summary report that contains the following information:
