Sabbatical Report. The academic staff member must prepare and forward to the member’s Xxxx a full written account of the member's scholastic and professional activities during the sabbatical. This report and details of the original sabbatical plan and any modifications to the plan, are to be included as part of the member’s Annual Information Form.

Sabbatical Report. A report of the work completed during the sabbatical will be provided to the Xxxx of the appropriate College at the conclusion of the sabbatical year. The report should contain a copy of the letter requesting sabbatical and a summary of accomplishments aligned to intended goals and objectives.
Sabbatical Report. Within 90 days of return from sabbatical, a Member will submit a report on the sabbatical to the Xxxx. The report will describe the activities undertaken during the sabbatical and will indicate to what extent the research goals were achieved or modified over the course of the sabbatical. Sabbatical Report. Not later than thirty (30) days upon completion of the sabbatical, a faculty member is required to submit to the Vice President of Academic and Student Affairs a final sabbatical report detailing the accomplishments/activities completed during the sabbatical. With the written approval of the Vice President of Academic and Student Affairs an interim report may be submitted within thirty (30) days, and a final report shall be submitted by an agreed upon date. The purpose of the sabbatical leave may not be changed from the purpose stated in the application without the written permission of the Vice President of Academic and Student Affairs.

The Academic Supervisor will evaluate the sabbatical activities by comparing the faculty member’s sabbatical proposal with the faculty member’s report.

  • MEDICAL REPORT The Agency/Department Head as a condition of granting sick leave with pay, may require medical evidence of sickness or injury acceptable to the Agency/Department. The acceptable medical evidence must be obtained from a medical practitioner currently treating the employee or the employee’s family member.

  • Final Report The goal of this subtask is to prepare a comprehensive Final Report that describes the original purpose, approach, results, and conclusions of the work performed under this Agreement. The CAM will review the Final Report, which will be due at least two months before the Agreement end date. When creating the Final Report Outline and the Final Report, the Recipient must use the Style Manual provided by the CAM.
