Record Transfer. Upon receipt of written request , PROVIDER shall transfer to the provider, designated in the request, copies of all medical records, and other data in the possession or control of PROVIDER pertaining to the covered consumer preferably within 24 hours, but not to exceed three (3) days of such notice. PROVIDER ▇▇▇▇ ▇▇▇▇▇▇▇, accept, and honor the approved MDHHS 5515 standard consent form to release SUD records.
