Common use of Record Transfer Clause in Contracts

Record Transfer. Upon receipt of written request from MSHN, PROVIDER shall transfer to PROVIDER, designated in the request, copies of all medical records, and other data in the possession or control of the PROVIDER pertaining to the covered consumer within three (3) days of such notice. PROVIDER ▇▇▇▇ ▇▇▇▇▇▇▇, accept, and honor the approved MDHHS 5515 standard consent form to release SUD records.
