Provisions re Plan A and Plan B Maternity Leave. a) At least two (2) weeks prior to returning to work following her Maternity Leave an employee shall notify the Fire Paramedic Chief of the date of her intended return and shall provide, to the City’s Occupational Health Physician, a Medical Consent Form in the format required by the Service, completed by her attending physician certifying that she is able to perform all her normal duties.
