Program Delivery. Subrecipient shall utilize its grant to provide the salary including fringe benefits for a Case Manager and a Business Manager. The case manager will help seniors meet their immediate needs and identify a path to build their self-sufficiency. The objectives for the program are: Objective #1 Develop a service plan for participating seniors with the assistance of the case maanger Objective #2 Participating seniors will complete the steps outlined in their service plan Objective #3 Participating seniors completing their service plan show an increase in self sufficiency Objective #4 Participating seniors will be referred to other programs that can meet their needs The Subrecipient, or its Representatives, shall be responsible for the integrated case management program hiring a part-time Case Manager.
