Privacy Notice Statement. Prior to collecting personal information or other information from consumers for purposes of fulfilling an authorized function, CDO must provide consumers with a privacy notice statement. The privacy notice statement must be in writing and must be provided on, or simultaneously with, any electronic and/or paper form CDO will use to gather and/or request personal information or other information from consumers. The privacy notice statement must also be prominently and conspicuously displayed on CDO’s public-facing website, if applicable, if the CDO will gather or request personal information or other information from consumers through that website.
