Personal Identification. If the Hospital requires employees to carry personal identification, the cost of said personal identification and related expense shall be borne by the Hospital. Worn I.D. will be replaced by the Hospital; employees who lose their I.D. will be required to pay $5.00 toward the replacement cost.
