PERFORMING AGENCY RESPONSIBILITIES. SCHOOL SURVEY PHASE ONE A. During Phase One, the Performing Agency, under the direction of the System Agency’s Program Project Manager (Project Manager), shall administer the School Survey. B. Conduct the School Survey in the winter and spring of the state fiscal year. The Performing Agency shall collect data to be used to produce statewide School Survey aggregate data and reports in the following Fiscal Year . The Performing Agency shall also provide incentives for the participating individual schools selected in the statewide sample. In addition, The Performing Agency shall conduct an over-sample of schools in 28 counties located along the Texas-Mexico border (Special Study Area) concurrent with the School Survey. The counties in the Special Study area are Brewster, Brooks, Cameron, Culberson, Dimmitt, Duval, Edwards, El Paso, Hidalgo, Hudspeth, ▇▇▇▇ ▇▇▇▇▇, ▇▇▇ ▇▇▇▇, ▇▇▇ ▇▇▇▇▇, ▇▇▇▇▇▇, ▇▇▇▇▇▇, ▇▇ ▇▇▇▇▇, ▇▇▇▇▇▇▇▇, Pecos, ▇▇▇▇▇▇▇▇, ▇▇▇▇▇▇, ▇▇▇▇▇, ▇▇▇▇▇▇▇, ▇▇▇▇▇▇, ▇▇▇ ▇▇▇▇▇, ▇▇▇▇, ▇▇▇▇▇▇▇, ▇▇▇▇▇▇, and ▇▇▇▇▇▇. C. In addition, and in conjunction with the statewide effort, the Performing Agency shall conduct a second over-sample of schools in each of the eleven state planning regions (Regional Study) concurrent with the School Survey for the purpose of providing regional reports in addition to a statewide report. The Performing Agency shall provide Regional Study aggregate data and reports and provide incentives for the participating individual schools in each region. It is possible that some collapsing of regions shall be required if there is insufficient participation. Regional data may also be collapsed if the standard error due to sampling in one or more of the regions is too high. Any collapse of target data will be approved in writing by the System Agency. D. The activities to be performed by the Performing Agency under this Contract detailed in the Tasks below are collectively referred to as the Project. The Project shall include printing of survey instruments and materials; use of incentives for the statewide School Survey and Regional Study recruitment; statewide School Survey and Regional Study administration; and data scanning and processing. The parties acknowledge that this agreement is not a sponsored research agreement or research contract or grant. E. All activities in this Project will be coordinated with the recruitment of schools and students for the Texas Youth Tobacco Survey (YTS). The work on the YTS will be done by the Performing Agency under a separate Contract. F. The Fiscal Year 2020 activities for the terms of September 1, 2019 through August 31, 2020 are documented as follows:

C. The School Project shall include printing of survey instruments and materials; use of incentives for the statewide School Survey and Regional Study recruitment; statewide School Survey and Regional Study administration; and data scanning and processing. The parties acknowledge that this agreement is not a sponsored research agreement or research contract or grant.The
E. All activities in this School Project will be coordinated with the recruitment of schools and students for the Texas Youth Tobacco Survey (YTS( YTS ). The work on the YTS will be done by the Performing Agency under a separate Contract.
F. The Fiscal Year 2020 FY 24 activities for the terms term of September 1, 2019 2023 through August 31, 2020 2024 are documented as follows:
Task 1: Prepare Written Progress Reports. Performing Agency shall submit a monthly progress report to both the Project Manager and the System Agency Substance Abuse Contracts Mailbox (▇▇▇.▇▇▇▇▇▇▇▇▇@▇▇▇.▇▇▇▇▇.▇▇▇) via email no later than the 15th day of each month. These reports shall:
i. Be written using the template provided by System Agency;
ii. Summarize School Survey activities during the prior completed month;
iii. Discuss School Survey activities planned for the following month;
iv. Outline delivery strategy; and
v. Communicate any obstacles that need to be addressed with the Project Manager and document in the monthly progress reports.
Task 2: Approval from Performing
i. Be responsible for ensuring any research involving human subjects related to the activities in this Contract is reviewed and approved by Performing ;
ii. Submit the School Survey finalized recruitment materials and instruments, based on preliminary submissions, to the IRB by September 15, 2023;
iii. Be responsible for investigating any allegations of scientific misconduct against its own regular or contract employees.
iv. Submit IRB approval documents no later than November 15, 2023 as the deliverable for this task.
i. Produce one survey form for grades seven (7) through twelve (12) in English and Spanish.
ii. Secure Scantron Corporation (or its subsequent company name should it change names or be acquired by, merged with, or otherwise affiliated with (an)other company(ies)) as the vendor for printing the survey instruments and ensure that a sufficient number of copies of the instruments are printed for the statewide School Survey and Regional Study.
iii. Prepare other materials that are part of the implementation (instructions, etc.) for the statewide School Survey and Regional Study. These materials must include a redesigned recruitment packet, full color pamphlet with Frequently Asked Questions, letters of support, and other recruitment materials determined as necessary by HHSC.
iv. School Project ( Project Website ) to provide information that supplements the information contained in the recruitment packet, including a PowerPoint presentation providing an overview of the School Survey and any additional content updates required by System Agency. The information provided on the Project Website will include copies of the campus coordinator responsibilities forms, survey coordinator responsibilities forms, and parental notification letters to facilitate survey administration.
v. Deliver School Survey instruments and related materials by January 1, 2023.
i. Implement the new statewide sampling strategy , which must first be reviewed and approved by System Agency. Overall, approximately 178 schools are targeted to participate in the statewide School Survey and Regional Study. Performing Agency shall cease recruitment of schools when the number of enrolled schools reaches 178. Performing Agency shall use the following as goals for campus participation rate:
