Outreach and Transition Planning. The CSP must cooperate with and assist SCDDO with outreach and transition planning procedures which identify, through the data collection system, the number of new persons who are likely to need services and what services they may likely need in the future. This activity may include providing case management to persons anticipating services from the CSP, attendance at Individual Education Plan (“IEP”) meetings and attendance at transition planning meetings.
