Other Provider Changes. In the event that an Enrollee is in an ongoing course of treatment with another Participating Provider who becomes unavailable to continue to provide SECTION 21 (PROVIDER NETWORK AND AGREEMENTS) October 1, 2004 21-6 services to such Enrollee, the Contractor shall provide written notice to the Enrollee within fifteen (15) days from the date on which the Contractor becomes aware of the Participating Provider's unavailability to the Enrollee. In such cases, the notice shall describe the procedures for continuing care consistent with subdivision 6 (e) of PHL Section 4403 and for choosing an alternative provider.
