Medical Note. (a) An employee absent for more than five (5) consecutive working days may be asked to furnish within seven (7) working days from commencement of absence, a medical note from her physician certifying the duration of illness. The seven (7)-day period may be extended by the Manager if the employee is incapacitated to the extent that she is unable to produce the medical note certifying the illness within that period. (b) An employee absent for more than twenty (20) consecutive working days shall: (i) provide immediately following such twenty (20) days, a medical note from her physician certifying the illness, the latest date the employee was seen by the physician and the probable date on which the employee will return to work; and (ii) provide further medical notes from her physician, covering the same information, following each subsequent forty (40) consecutive working days of absence.
