LEAVE APPLICATION. Except as otherwise indicated in this section, application for a leave of absence or extension thereof, shall be made in writing to the Superintendent at least thirty (30) days prior to the proposed commencement of the leave, except in serious and unusual circumstances.

LEAVE APPLICATION. 39.1 An Employee shall, where possible, apply and receive prior authorization by the Department Head or designate for all leave, failing which an Employee shall be considered absent without permission.

LEAVE APPLICATION. An application for Wage Continuance Benefits must be completed for all medical absences by the employee and forwarded to the management representative as determined by the Employer and communicated to employees. If the duration of the absence is five (5) days or less, this application may be submitted upon the employee’s return to work. If the absence is expected to be greater than five (5) working days, the employee should make every effort to submit an application within the first week of absence.

  • Maternity/Paternity/Adoption Leave An Employee who is expecting the birth or adoption of a child shall be entitled to maternity/paternity/adoption leave without pay, provided she presents a medical certificate confirming the probable date of confinement, or in the case of adoption, gives the Employer notice of eligibility. Except in extenuating circumstances, the notice shall be submitted in writing at least twenty-eight (28) days in advance of the leave and shall specify the probable date of commencement and the anticipated length of leave.
