Name, address, and telephone number of Contractor, and the address to which payments are to be sent, if different from the Contractor’s address; 2. DSHS Contract Number; 3. Identification of service(s) or goods provided; 4. Dates services or goods were delivered; 5. Total invoice amount; and 6. Any additional payment information that is required by the Contract or DSHS.
