Common use of HIPAA Addendum Clause in Contracts

HIPAA Addendum. This addendum (“Addendum”) will take effect on the date prescribed by the first paragraph of the DME and Related Services Agreement dated as of March _____, 2005 (the “Agreement”) by and between OrthoSupply Management, LLC (“OSMI”) and Island Orthopaedics & Sports Medicine, P.C. (the “Provider”) in order to comply with the requirements of the Health Insurance Portability & Accountability Act of 1996 (“HIPAA”) and its implementing regulations (45 CFR 160-164 as amended from time to time, the “Privacy Rules”).

(the “Provider”) in order to comply with the requirements of the Health Insurance Portability & Accountability Act of 1996 (“HIPAA”) and its implementing regulations (45 CFR 160-164 as amended from time to time, the “Privacy Rules”).
