Evidence Required. Prior to execution of the Agreement, the Consultant shall file with the City evidence of insurance from an insurer or insurers certifying to the coverage of all insurance required herein. Such evidence shall include original copies of the ISO CG 00 01 (or insurer’s equivalent) signed by the insurer’s representative and Certificate of Insurance (Acord Form 25-S or equivalent), together with required endorsements. All evidence of insurance shall be signed by a properly authorized officer, agent, or qualified representative of the insurer and shall certify the names of the insured, any additional insureds, where appropriate, the type and amount of the insurance, the location and operations to which the insurance applies, and the expiration date of such insurance.
Evidence Required. On the Effective Date of this Agreement, ELI shall provide BPA with a certificate of insurance (Certificate of Insurance) executed by an authorized representative of the insurer(s) evidencing that ELI insurance complies with this section. A copy of all required endorsements shall be attached to and form a part of the Certificate of Insurance.
Evidence Required. Within ten (10) working days following receipt of notice that a contract has been awarded, Contractor shall have provided the MTS Contracts Specialist with satisfactory certification by a qualified representative of the Insurer(s) that Contractor's insurance complies with all provisions in this insurance section.
Evidence Required. The Fire Chief may require evidence as to the adequacy of the reason for any employee's absence during the time for which sick leave is requested. However, no restrictions shall be placed upon the employee's activity during off duty time (i.e., time during which the employee is not scheduled to be at work).
Evidence Required. At or before execution of this Agreement and at such other times as BART may request, Franchisee shall provide BART with a Certificate of Insurance executed by an authorized representative of the insurer(s) evidencing that Franchisee’s insurance complies with this article. The Certificate shall reference the BART Agreement Number and Title to which the certificate relates. In addition, a copy of all required endorsements shall be attached to and form a part of Franchisee’s Certificate of Insurance.
Evidence Required. When an employee is required to bring evidence to Court, the start of overtime is generally when the employee checks out the required evidence. When an employee is required to return evidence not admitted into Court, end of overtime is generally at the time and location where the evidence is returned.
