Evidence Required. Prior to execution of the Agreement, the Consultant shall file with the City evidence of insurance from an insurer or insurers certifying to the coverage of all insurance required herein. Such evidence shall include original copies of the ISO CG 00 01 (or insurer’s equivalent) signed by the insurer’s representative and Certificate of Insurance (Xxxxx Form 25-S or equivalent), together with required endorsements. All evidence of insurance shall be signed by a properly authorized officer, agent, or qualified representative of the insurer and shall certify the names of the insured, any additional insureds, where appropriate, the type and amount of the insurance, the location and operations to which the insurance applies, and the expiration date of such insurance.

All evidence of insurance shall be certified by a properly authorized officer, agent or qualified representative of the insurer and shall certify the names of the insured, any additional primary insureds, where appropriate, the type and amount of the insurance, the location and operations to which the insurance applies, and the expiration date of such insurance.
Evidence Required. The Fire Chief may require evidence as to the adequacy of the reason for any employee's absence during the time for which sick leave is requested. However, no restrictions shall be placed upon the employee's activity during off duty time (i.e., time during which the employee is not scheduled to be at work).
Evidence Required. When an employee is required to bring evidence to Court, the start of overtime is generally when the employee checks out the required evidence. When an employee is required to return evidence not admitted into Court, end of overtime is generally at the time and location where the evidence is returned.

Within ten (10) working days following receipt of notice that a contract has been awarded, Contractor shall have provided the MTS Contracts Specialist with satisfactory certification by a qualified representative of the Insurer(s) that Contractor's insurance complies with all provisions in this insurance section.

Evidence Required. A manager may request that you provide medical evidence or a statutory declaration for any period of personal leave. Medical evidence, or a statutory declaration, is required where the number of days of personal leave taken in twelve month period exceeds five days. No more than two consecutive days of personal leave may be taken without medical evidence or statutory declaration.
Evidence Required. When taking sick leave or carer’s leave, to be entitled to payment the employee must provide evidence of the reason for the absence by producing either: • A certificate from a registered health practitioner (as defined in the Workplace Relations Act), or • A statutory declaration, only if it is not reasonably practicable for the employee to provide a medical certificate. In each anniversary year, an employee is entitled to be absent from work for two single days or equivalent part days, without production of a medical certificate and/or statutory declaration. Such entitlement is subject to other conditions relating to payment of leave being met, including the requirement to give notice. Such single days cannot be consecutive. Additional provisions relating to evidence of the reason for absence apply in certain circumstances, as specified below:
