Evidence Required. Prior to execution of the Agreement, the Consultant shall file with the City evidence of insurance from an insurer or insurers certifying to the coverage of all insurance required herein. Such evidence shall include original copies of the ISO CG 00 01 (or insurer’s equivalent) signed by the insurer’s representative and Certificate of Insurance (Xxxxx Form 25-S or equivalent), together with required endorsements. All evidence of insurance shall be signed by a properly authorized officer, agent, or qualified representative of the insurer and shall certify the names of the insured, any additional insureds, where appropriate, the type and amount of the insurance, the location and operations to which the insurance applies, and the expiration date of such insurance.

Evidence Required. A manager may request that you provide medical evidence or a statutory declaration for any period of personal leave. Medical evidence, or a statutory declaration, is required where the number of days of personal leave taken in twelve month period exceeds five days. No more than two consecutive days of personal leave may be taken without medical evidence or statutory declaration.
Evidence Required. When taking sick leave or carer’s leave, to be entitled to payment the employee must provide evidence of the reason for the absence by producing either: • A certificate from a registered health practitioner (as defined in the Workplace Relations Act), or • A statutory declaration, only if it is not reasonably practicable for the employee to provide a medical certificate. In each anniversary year, an employee is entitled to be absent from work for two single days or equivalent part days, without production of a medical certificate and/or statutory declaration.
