Episode of Care. All clinically related services for one patient for a discrete diagnostic condition from the onset of symptoms until treatment is complete. For individuals with a mental health diagnosis, an Episode of Care for an outpatient service with an Out-of-Network Provider will not exceed two (2) years from the date of Enrollment. For inpatient care, the Episode of Care is complete at discharge.
Episode of Care. Nurse triages Xxxxxxx in remote assessment centre using clinical decision support software as urgent (requires to be seen within 2 hours) and an appointment is booked in the local after hour’s service unit for 1900hrs. Xxxxxxx is seen by the Registered Nurse Prescriber at 1910 hrs and following a comprehensive assessment is diagnosed with a urinary tract infection. The nurse prescribes Trimethoprim 100mgs PO for 7 days and then prints the prescription on the dot-matrix printer which is used to address patient care reimbursement scheme (PCRS) which is in operation in Ireland.
Episode of Care. EIS Coordinators shall complete an Episode of Care for each Client to track linkage to medical care and case management and to document Client education concerning the HIV disease process, risk reduction, maintenance of the immune system, disclosure/support, and importance of adherence to treatments and medications.
