Eligibility and Application Process. (a) Employees making application must have completed two (2) continuous years of employment at Yukon College. The Employer shall not be required to grant leave during the same period of time to more than three (3) employees per division. An interested employee must make written application no later than May of each year. Such written applications are to be directed to the Human Resource Services department. The Employer will respond to applications by June Iof each year. Such responses will be in writing and shall clearly indicate acceptance or denial. The approval of July June individual requests to participate in the plan rests solely with the employer. Such approval shall not be unreasonably withheld. Contract All employees wishing to participate in the Plan shall be required to sign the approved contract before approval for participation is granted. Payment Formula
Eligibility and Application Process. 2.1 If you pay Council Tax to either Dartford Council or Thurrock Borough Council, you may be eligible for a Local Residents’ Discount. You must prove to our reasonable satisfaction that you pay Council Tax to Dartford or Thurrock Borough Councils and that you own each of the vehicles covered in your application. Only one vehicle per household member is permitted to receive the discount, however all household members are eligible.

Gov’t Code § 2274.001(3) of SB 19.

  • How to File an Appeal of a Prescription Drug Denial For denials of a prescription drug claim based on our determination that the service was not medically necessary or appropriate, or that the service was experimental or investigational, you may request an appeal without first submitting a request for reconsideration. You or your physician may file a written or verbal prescription drug appeal with our pharmacy benefits manager (PBM). The prescription drug appeal must be submitted to us within one hundred and eighty (180) calendar days of the initial determination letter. You will receive written notification of our determination within thirty (30) calendar days from the receipt of your appeal. How to File an Expedited Appeal Your appeal may require immediate action if a delay in treatment could seriously jeopardize your health or your ability to regain maximum function, or would cause you severe pain.
