District Form Clause Samples
The District Form clause specifies the standardized format or template that must be used for documents, submissions, or communications within a particular district or jurisdiction. In practice, this clause requires parties to use official forms provided by the district, such as for permit applications, compliance reports, or legal filings, ensuring that all necessary information is included and presented in a consistent manner. District Form. A written evaluation will be made on the prescribed District performance evaluation forms provided by the Office of Human Resources.
(i) The form will be completed and signed by the evaluator and the unit member.
(ii) A copy shall be given to the unit member at the time of the conference.
(iii) The original shall be transmitted by the supervisor or manager to the next higher person in administrative channels for review and comments.
