Determining Service. The Senate President will conduct an open call for applicants for the next Co- Facilitators from the entire full-time faculty Spring 2022 unless the group has agreed to extend the duration of the Co-Facilitators' term. The Senate Executive and TLC Co- Facilitators will work collaboratively to select new Co-Facilitator(s). Previous experience with TLC is preferred. The term of the incoming Co-Facilitator(s) begins fall 2022 for onboarding with current Co-Facilitators. The Senate Executive will notify the appropriate ▇▇▇▇ and VP for scheduling purposes. Duties & Deliverables: The role of Teaching Learning, and Community (TLC) Co-Facilitator includes but is not limited to: •Guiding Professional Development activities across the entire campus for all employees; • Working with a Co-Facilitator to plan, facilitate and evaluate regular TLC events such as Instructional Skills Workshop, New Employee Welcome, Part-Time Faculty In-Service, College In-Service, and Flex Day • Guiding the yearly cycle of planning, budgeting, and reporting TLC activities; • Documenting the TLC planning cycle, cataloguing documents,and codifying Executive processes; • Sustaining the inclusive and holistic vision of TLC; • Meeting regularly with the administration team, tri-chair facilitators, and other constituents to facilitate funding, reporting, and process development for TLC projects; • Scanning other institutions and resources for strategies and information thatmay be useful for Columbia College and sharing them; • Organizing and supporting faculty cohorts; • Participating on Institutional Effectiveness Council and Diversity, Equity, and Inclusion Committee; • Generating an annual report; • Creating a campus wide Professional Development plan; • Training new TLC Co-Facilitator(s).

Determining Service. For the purpose of this policy, continuous service shall be deemed to include – a) any period of absence from duty on annual leave, long service leave, paid compassionate leave, accrued paid personal leave and public holidays; b) any period of authorised paid absence from duty necessitated by sickness of or injury to the employee up to a maximum of three months in each calendar year, but not including leave without pay or parental leave; or c) any period of absence that has been supported by an approved workers compensation claim up to a maximum absence of 12 months. For the purpose of this policy, continuous service shall exclude – a) any period of unauthorised absence from duty unless the CEO determines otherwise; b) any period of unpaid leave unless the CEO determines otherwise; or c) any period of absence from duty on parental leave unless the CEO determines otherwise.
Determining Service. The Elections Officer will issue a call for applicants for the SLOPR coordinator to the entire full-time faculty in Spring 2022. The candidate will provide a statement detailing their previous experience with SLOs and Program Review. These candidate statements shall be forwarded to the Elections Officer within the time-frame outlined in the Elections Procedures. Ideally, the beginning of the two- year term will be a different year than the beginning of the Curriculum Chair term. Terms begin after Spring graduation. DUTIES & DELIVERABLES: The role of the SLOPR Data Coordinator includes but is not limited to: • Co-Chair SLOPR Committee meetings (with Vice President of Instruction (VPI)) • Create SLOPR Committee agenda (with VPI) • Attend Institutional Effectiveness Council (IEC) meetings: Report for SLOPR Committee and carry recommendations between IEC and SLOPR regarding SLOs and Program Review. • Ensure that the college’s departments continue to follow the established Synchronized Program Review and SLO Cycles (see graphic at bottom of this MOU)
Determining Service. The Election's Officer will issue a written call for nominations for the Curriculum Committee Chair to theCurriculum Committee in Fall 2023. • The Curriculum Committee shall nominate one or more candidates for Curriculum Committee Chair from the current voting membership of the committee or a past member who has served a complete three year term. These nominations shall be forwarded to the Election’s Officer within the timeframe outlined in the Election Policies and Procedures document. • Terms begin after Spring graduation. DUTIES & DELIVERABLES The role of the Curriculum Chair includes but is not limit ed to: • Propose an annual calendar and schedule of Committee meetings to the Executive Board; • Solicit agenda items and set agendas; • Attend meetings of the Curriculum Committee and its sub-committees or arrange for an alternate when necessary; • Attend Academic Senate meetings and provide reports to the Senate; • Serve on Senate Executive; • Facilitate Committee deliberations; • Streamline and facilitate the process of curriculum development and maintenance; • Forward Committee recommendations to the Academic Senate President; • Ensure training of those individuals new to the Curriculum Committee, including the use of eLumen and the approval processes; • Sign annual CCCCO Certification form each fall. • Ensure Course Stand-Alone trainings to curriculum members at the beginning of each academic year. Special Notes: The performance of these duties may be evaluated as part of a faculty member's regular evaluation process. This MOU may be revised by mutual agreement between the College President and the Academic Senate based on college needs. Through a special meeting, Senate Executive to grant approval of temporary or permanent change in Curriculum Chair. • Curriculum Committee will recommend a temporary or permanent replacement for the existing Curriculum Chair and share the recommendation with Senate Executive Committee

  • Standards of Service In connection with the performance of any Services pursuant to this Agreement: 7.1 Number of Employees Contractor warrants it will provide sufficient employees to complete the Services ordered within the applicable time frames established pursuant to this Agreement. Furthermore, Contractor shall, at its expense, supply all tools, equipment, and other materials necessary to perform the Services contemplated in this Agreement. 7.2 Skill of Employees Contractor warrants that employees shall have sufficient skill, knowledge, and training to perform the Services and that the Services shall be performed in a professional and workmanlike manner.

  • Standard of Service As Agent for the Fund, you agree to provide service equal to or better than that provided by you or others furnishing shareholder services to other open-end investment companies ("Standard") at a fee comparable to the fee paid you for your services hereunder. The Standard shall include at least the following: (a) Prompt reconciliation of any differences as to the number of outstanding shares between various Facility records or between Facility records and records of an MFS Fund's Custodian; (b) Prompt processing of shareholder correspondence and of other matters requiring action by you; (c) Prompt clearance of any daily volume backlog; (d) Providing innovative services and technological improvements; (e) Meeting the requirements of any governmental authority having jurisdiction over you or the Fund; and (f) Prompt reconciliation of all bank accounts under your control belonging to the Fund or MFS. If any MFS Fund serviced by you is reasonably of the view that the service provided by you does not meet the Standard, it shall give you written notice specifying the particulars, and you then shall have 120 days in which to restore the service so that it meets the Standard, except that such period shall be 180 days with respect to meeting that portion of the Standard described above in item (d) of this paragraph 4. If at the end of such period the Fund remains reasonably of the view that the service provided by you, in the particulars specified, does not meet the Standard, then the MFS Fund or Funds having a majority of the accounts for which you are then Agent may, by appropriate action (including the concurrence of a majority of the Trustees or Directors, as the case may be, of such MFS Fund or Funds who are not interested persons of MFS), elect to terminate this Agreement for cause as to all such Funds upon 90 days notice to you. Upon termination hereof, the Fund shall pay you such compensation as may be due to you as of the date of such termination, and shall likewise reimburse you for any costs, expenses, and disbursements reasonably incurred by you to such date in the performance of your duties hereunder.

  • TESTING SERVICES DESCRIPTION This Exhibit contains additional terms and conditions applicable to testing services for digestive disorders (the “Testing Services”) that you may have purchased. The additional terms and conditions in this Exhibit only apply to the services described in this Exhibit.

  • Level of Service Complex case management with a provider focus is appropriate for members who either choose not to be actively involved or are unable to actively participate in their health care. Complex case management targets members with two (2) or more disease states who need assistance with care coordination, making preventive care appointments, or accessing care to address the members’ chronic health conditions or members who have had an inpatient hospital stay in the last ninety (90) days or members with high dollar claims of over fifty thousand dollars (>$50,000) in six (6) months. The focus is on working with the providers to meet the needs of the individual through communication with the PMP (if applicable), other providers, and the member’s natural support system. The goal is to help members gain optimum health or improved functional capability, in the right setting and in a cost-effective manner. Complex case management with provider focus is the active coordination by the Contractor of care and services between providers while navigating the extensive systems and resources required for the member. It involves comprehensive assessment, determination of available benefits, development and implementation of a complex case management plan directed at the chronic health conditions. At a minimum, the Contractor must provide complex case management services for members discharged from an inpatient psychiatric, drug overdose, or substance abuse hospitalization, for no fewer than ninety (90) calendar days following that inpatient hospitalization discharge. The Contractor must also provide complex case management services for any member at risk for inpatient psychiatric or substance abuse re -hospitalization. Care managers must contact members during an inpatient hospitalization or as soon as practicable upon receiving notification of a member’s inpatient behavioral health hospitalization. The care manager must work with the hospital discharge planner, provider case manager and/or natural supports (i.e. family) to ensure that an outpatient follow-up appointment is scheduled to occur no later than seven (7) calendar days following the inpatient behavioral health hospitalization discharge and transportation is not a barrier to attending the appointment. Complex case management with provider focus includes all of the services and benefits from disease management and care management. In addition,

  • Cost of Services Except to the extent specified otherwise in the Agreement, all costs of performing the Services are included in the Contract Price and TOMRA shall not be entitled to any further payment in respect thereof.
