Delivery of Services to Eligible Clients. The Sub-Recipient shall ensure the provision of services to address the diverse needs of functionally impaired elders. The Sub-Recipient shall not provide direct services to clients, unless specifically authorized in proviso. The Sub-Recipient shall ensure performance and reporting of the following services per the Sub-Recipient’s current AAAPP-approved application, the current DOEA Handbook, and Section II.A.1-3 of this contract. Documentation of service delivery must include a report consisting of the following: number of clients served, number of service units provided by service, and rate per service unit with calculations that equal the total invoice amount. The services include the following: Congregate Meals Home Delivered Meals
