INDIVIDUALS TO BE SERVED. In order to receive services under this contract, an individual must meet the following specific program eligibility requirements:
INDIVIDUALS TO BE SERVED. Homeless Mentally Ill Adults living in COUNTY, age eighteen (18) and over, who may also have a co-occurring substance abuse disorder. Referrals will come from a number of sources, including but not limited to, the following:
INDIVIDUALS TO BE SERVED. 1.4.1 Eligibility for Program To be eligible for services under this contract, an individual must meet the following criteria: (1) Be 65 years of age or older. (2) Have Medicare Parts A & B as reflected in the Florida Medicaid Management Information System (FMMIS). (3) Be Medicaid eligible with incomes up to the Institutional Care Program (ICP) level. (4) Reside in the project service area. (5) Be determined by CARES to be at risk of nursing home placement and meet one or more of the following clinical criteria: a. Require some help with five or more activities of daily living (ADLs); or b. Require some help with four ADLs plus requiring supervision or administration of medication; or c. Require total help with two or more ADLs; or d. Have a diagnosis of Alzheimer’s disease or another type of dementia and require assistance or supervision with three or more ADLs; or have a diagnosis of a degenerative or chronic condition requiring daily nursing services; or e. Have a diagnosis of a degenerative or chronic condition requiring daily nursing services. (6) Be determined by CARES to be a person who, on the effective date of enrollment, can be safely served with home and community-based services. 1.4.2 Ineligibility for the Program The following persons are ineligible for the program: (1) Persons residing outside the project service area. (2) Persons residing in a state hospital, intermediate care facility for persons with developmental disabilities, or a correctional institution. (3) Persons participating in or enrolled in another Medicaid waiver project on the effective date of enrollment. (4) Medicaid eligible recipients who are served by the Florida Assertive Community Treatment Team (FACT team). (5) Persons enrolled in any other Medicaid capitated long-term care program, in a Medicaid HMO, MediPass program, on the effective date of enrollment.
INDIVIDUALS TO BE SERVED. The target population for the Adult and Older Adult Housing Navigation Services program consists of homeless adults, or those at risk of homelessness, residing in COUNTY, eighteen to fifty-nine (18 to 59) years of age, and older adults sixty (60) years of age and above, who have been diagnosed with a serious mental illness (SMI) and who may have a co- occurring disorder. The CONTRACTOR will work in collaboration with various County partners such as the Coordinated Entry System, Housing and Residential Care department, HCA Outreach and Engagement, and Adult and Older Adult Behavioral Health (AOABH) treatment providers to secure a wide range of housing options for clients who are ready to live independently in the community. Clients must be Medi-Cal beneficiaries, and will be connected with the service provider (Case Manager/Plan Coordinator/Personal Service Coordinator) as well as a peer mentor for support to sustain housing. Individuals placed in housing through the Housing Navigators but not yet interested in services will be connected with O&E and a peer mentor for ongoing support.
INDIVIDUALS TO BE SERVED. CONTRACTOR shall provide short term crisis residential 16 services to individuals evaluated by and referred by COUNTY and COUNTY contractors as 17 appropriate. CONTRACTOR shall not provide walk-in evaluation and admission services unless 18 mutually agreed upon, in writing, between CONTRACTOR and ADMINISTRATOR.

  • POPULATION TO BE SERVED A. In accordance with the Contract, Contractor is required, within the limits of the Contractor’s service capacity, to serve individuals who meet the financial and clinical eligibility criteria for an adult who is Seriously Mentally Ill (SMI) and is eligible for services as described in the DARMHA manual.
