Delivery of Services to Eligible Clients. The Sub-Recipient shall ensure the provision of a continuum of services that meets the diverse needs of elders and their caregivers. Documentation of service delivery must include a report consisting of the following: number of clients served, number of service units provided by service, and rate per service unit with calculations that equal the total invoice amount. The Sub-Recipient shall ensure the performance and reporting of the following types of services, in accordance with the Sub-Recipient’s current AAAPP- approved application, the current DOEA Programs and Services Handbook, and Section II.A.
