Common use of Client Plans Clause in Contracts

Client Plans. A. County requires client plans meet the following requirements: 1. Have specific observable and/or quantifiable goals identified in cooperation with the client. 2. Identify the proposed type(s) of intervention. 3. Have a proposed duration of intervention(s). 4. Are signed by: a. The person providing the service(s), or b. A person representing a team or program providing services, or c. A person representing the Contractor providing services. d. The client, except when client refuses or is unavailable. 5. The Client Plan must be signed or co-signed by one of the following approved staff categories: a. Licensed Physician b. Licensed/Waivered Psychologist c. Licensed/Waivered Clinical Social Worker d. Licensed/Waivered Marriage, Family and Child Counselor
