Common use of Case Management Services Clause in Contracts

Case Management Services. If the Project Sponsor is using Grant Funds to provide case management services, the Project Sponsor shall ensure that qualified facility- based agencies are contacted to provide individuals with housing prior to utilizing lodging facilities, such as hotels, motels, and other similar establishments, to provide housing. Proof of such attempts shall be submitted with the request for reimbursement as described in Article III, Section 3 herein. Project Sponsors providing case management shall also comply with the attached Scope of Services.
