AVAILABILITY OF PHI. In accordance with the Regulations, Business Associate shall Make available the information required to provide an Accounting of Disclosures of PHI as soon as possible, but in all cases no later than twenty (20) days from request by MetroHealth; For each Accounting of Disclosures, shall provide: The date of each disclosure; The name and address of the organization or person who received the PHI; A brief description of the information disclosed; For disclosures other than those made at the request of the subject, the purpose for which the information was disclosed, and a copy of the request or authorization for disclosure; and, Any other information that is required under the Regulations.
