Common use of Authorization to Take Resident’s Photograph Clause in Contracts

Authorization to Take Resident’s Photograph. By execution of this Agreement, the Resident, Resident Representative and/or Sponsor authorize the Facility to photograph the Resident for identification purposes and to photograph any part of the Resident to document certain physical conditions, e.g., wounds or skin discolorations, for treatment purposes. I understand that the Facility retains ownership rights to these photographs but that the Resident will be allowed access to view them or obtain copies.

Authorization to Take Resident’s Photograph. By execution of this Agreement, the Resident, Resident Designated Representative and/or Sponsor authorize the Facility to take and keep a photograph of the Resident for identification purposes and to photograph any purposes. All such photographs shall become part of the Resident to document certain physical conditions, e.g., wounds or skin discolorations, for treatment purposes. I understand that Resident’s file at the Facility retains ownership rights to these photographs but that the Resident will be allowed access to view them or obtain copiesFacility.

All such photographs shall become part of the Resident’s medical record at the Facility. The Resident to document certain physical conditions, e.g., wounds or skin discolorations, and Designated Representative authorize the taking of photographs of the Resident for treatment identification purposes.
