Applicant’s Responsibility. The applicant shall apply in writing to the Chief Human Resources Officer or designee in the following manner:
a. Request the leave by writing a narrative statement when it is apparent that the applicant’s existing leave will be exhausted before the applicant is able to return to work or is able to return from care of the family member.
b. Attach a physician’s statement that the applicant is unable to work due to an extended medical condition that incapacitates him or her and that the condition is likely to last for a specified period of time or that the applicant’s family member suffers from an incapacitating illness or injury. The District may require the applicant who is also an employee to go to a physician of its choosing for this statement.
c. An ill or injured employee may designate a family member or a CSEA representative to file a written request on his/her behalf.
Applicant’s Responsibility. The Applicant requesting a service license under the USWA:
A. must make application for a license to inspect, sample, classify, and/or weigh grain as directed by AMS. Each application shall:
(1) be signed by the applicant;
(2) be certified by a statement from the Warehouse Operator that the applicant is competent and authorized to perform the duties of a licensed inspector, sampler, classifier, and/or weigher;
(a) If the applicant is seeking an inspection license, the Warehouse Operator or authorized agent shall certify that such applicant can correctly inspect, sample, and classify grain in accordance with the United States Standards or, in the absence of such standards, in accordance with standards accepted by AMS.
(b) If the applicant is seeking a weigher’s license, the Warehouse Operator or authorized agent shall certify that such applicant can correctly weigh grain.
(3) furnish such additional information as requested by AMS.
B. may be required to submit to an examination or test to demonstrate the ability to properly inspect, sample, classify, and/or weigh grain in accordance with the United States Standards.
C. make available for inspection the equipment used or to be used to perform such duties.
Applicant’s Responsibility. The unit member who is eligible for the use of catastrophic leaveshall apply in writing to the Vice Chancellor, Human Resources, or designee in the following manner:
1. The request for leave shall be in writing and submitted to the Vice Chancellor, Human Resources, or designee, when it is apparent to the unit member that their existing leave shall be exhausted before they shall be able to return to work.
2. The requesting unit member shall attach a physician’s statement verifying that the unit member is unable to return to work due to their medical condition and/or the medical condition of the family member. The statement must also verify that the unit member’s condition shall likely continue to incapacitate the unit member for an extended period of time.
Applicant’s Responsibility. Applicant is responsible for preparing and/or approving the final text of the Credit as issued by Bank, irrespective of any assistance Bank may provide such as drafting or recommending text or by Bank’s use or refusal to use text submitted by Applicant. Applicant understands that the final form of any Credit may be subject to such revisions and changes as are deemed necessary or appropriate by Bank and Applicant hereby consents to such revisions and changes. Applicant is solely responsible for the suitability of the Credit for Applicant’s purposes. Applicant will examine the copy of the Credit and any other documents sent by Bank in connection with the Credit and shall promptly notify Bank of any non-compliance with Applicant’s Instructions and of any discrepancy in any document under any presentment or other irregularity. Applicant understands and agrees that Bank is not required to extend the expiration date of any Credit for any reason, and with respect to any Credit containing an “automatic amendment” to extend the expiration date of such Credit, Bank, in its sole and absolute discretion, may give notice of nonrenewal of such Credit and, if Applicant does not at any time want such Credit to be renewed, Applicant will so notify Bank at least fifteen (15) calendar days before Bank is required to notify the beneficiary of such Credit or any advising bank of such nonrenewal pursuant to the terms of such Credit.
Applicant’s Responsibility. Each Applicant is responsible for preparing and/or approving the final text of any Credit Issued by Bank for its account, irrespective of any assistance Bank may provide such as drafting or recommending text or by Bank’s use or refusal to use text submitted by such Applicant. Each Applicant is solely responsible for the suitability of the Credit for such Applicant’s purposes. Each Applicant will examine the copy of each Credit Issued for its account and any other documents sent by Bank in connection with such Credit and shall promptly notify Bank of any non-compliance with such Applicant’s Instructions and of any discrepancy in any document under any presentment or other irregularity. Each Applicant understands that the final form of any Credit may be subject to such revisions and changes as are deemed necessary or appropriate by Bank in accordance with standard industry practice and such Applicant hereby consents to such revisions and changes. Each Applicant understands and agrees that Bank is not required to extend the expiration date of any Credit Issued for its account for any reason and, with respect to any such Credit containing an “automatic amendment” to extend the expiration date of such Credit, Bank, in its sole and absolute discretion, may give notice of nonrenewal of such Credit at least five (5) calendar days (or such other period as shall be stated in such Applicant’s Application) prior to the end of the original term or, as the case may be, the period of extension. If such Applicant does not at any time want such Credit to be renewed, such Applicant will so notify Bank at least fifteen (15) calendar days before Bank is required to notify the beneficiary of such Credit or any advising bank of such nonrenewal pursuant to the terms of such Credit.
Applicant’s Responsibility. In accordance with PG&E's design, specifications, and requirements for the installation of Service Extensions generated by a Standalone Power System, subject to PG&E's inspection and approval, Applicant is responsible for:
1) Space for Standalone Power System Equipment. Applicant shall provide space, if needed, on Applicant's Premises at a location approved by PG&E for a standard Standalone Power System installation. Easements may be required by PG&E to install the Standalone Power System on Applicant’s Premises to serve only the Applicant. If the Standalone Power System Equipment will be or is designed to serve other third party properties, then PG&E may at its option install such equipment after appropriate rights of way or easements are obtained, satisfactory to PG&E. Detailed information on PG&E's requirements for the Standalone Power System will be furnished by PG&E.
Applicant’s Responsibility. Before submitting an application, each applicant is required to carefully examine the application and to completely familiarize themselves with all the terms and conditions. Ignorance on the part of the applicant will in no way relieve them of any of the obligations and responsibilities.
