After Care Clause Samples
After Care. The School provides written recommendations for follow-up services in the form of a discharge summary and is available for consultation and to assist with after-care services.
After Care. Program operates from 2:29 p.m. to 6:00 p.m. each school day. A late pick up fee of $1.00 per minute after 6:00 p.m. will be charged. A child may be terminated from the Program if they are picked up after 6:15 p.m. more than three times a year. On Tuesdays, due to banked time days, the program operates from 1:29 p.m. to 6:00 p.m.
After Care. The school offers an After Care Program for students enrolled in St. ▇▇▇▇▇▇ School on their attendance days. This program offers a safe, structured program for our students after school from 3:00– 6:00 p.m. Student behavior expectations in the After Care Program are the same as for the regular school program. Safety for all is of prime importance. Any student not picked up by 3:10 p.m. will be sent to After Care and billed accordingly. Electronics are not allowed in After Care unless being used for educational purposes, which must be approved by a staff member. Students must be signed out by parent or guardian and will be dismissed at door number #14. Parents may park on angled spots (after 3:15pm) on Stone Avenue or school parking lot. After Care. Once released to return to work, the Employee will be allowed three (3) hours or less for any doctor recommended after care appointments/treatment related to the occupational injury without affecting his regular pay. The Employee will notify his Supervisor of all appointments at least twenty-four (24) hours in advance and will provide documentation of the appointment upon his return to work.
After Care. Guidelines from the FSRH are that it is no longer a requirement to conduct a follow up review with the patient after the fitting of an IUCD providing adequate information has been provided. Surrey County Council expects that each patient is assessed by the clinician fitting the device as to the level of after care that may be needed and acts appropriately.
After Care. After care instructions must be followed explicitly, whether given in writing or orally. Failure to follow after care instructions may compromise the final results of the treatment.

After Care. Do not fertilize at planting time. Water regularly throughout the first growing season (about once a week unless significant rainfall is received), but do not overwater. Keep lawn mowers and string trimmers away from tree to avoid wounding bark. Only prune dead or injured branches at time of planting. Do not plant flowers or cultivate soil directly under tree.
After Care. Provide individualized discharge planning to include the following 8 after care services:
9 7.14.1 In-home staffing support, as needed, for at least thirty 10 (30) days after the Youth/NMD exits the program;
11 7.14.2 Twenty-four hours/seven days per week (24/7) crisis 12 intervention on-call phone support for at least thirty (30) days after the 13 Youth/NMD exits the program;
14 7.14.3 Time-limited, purposeful therapeutic intervention and 15 support to ensure the Youth’s/NMD’s successful transition to a lower level of 16 care; and
After Care. 15.1 The services we provide to you are on a fee for service basis and while we care about you and your individual wellness journey, you acknowledge and agree that no after care follow up contact will be made by us, unless we have specifically agreed in writing for the continued provision of services or after care.
15.2 You agree that all support, including follow up contact, ends at the conclusion of your last session with us. You further agree that the absence of aftercare follow up contact is no reflection on the quality or level of care provided by our service.
