Accountability Provision – Monitoring and Revocation. Practitioner and Healthways acknowledge the obligation and authority of Group Health Cooperative and CMS to monitor the performance and revoke where necessary any applicable activities performed under the Agreement if a determination is made that activities as defined in the Agreement are unsatisfactory as outlined in 422.504(i)(3)(ii); 422.504(i)(4)(ii)-(iii).
