Common use of Access to PHI Clause in Contracts

Access to PHI. 2.6.1. Business Associate shall, at the request and direction of Covered Entity, make PHI maintained by Business Associate in a Designated Record Set available to Covered Entity, or, as directed by Covered Entity, to the individual identified as being entitled to access and copy such PHI, within five (5) business days of receipt of such a request from Covered Entity.

Access to PHI. 2.6.1. If Business Associate shall, at the request and direction of Covered Entity, make maintains PHI maintained by Business Associate in a Designated Record Set available to for Covered Entity, Business Associate must provide access, at the request of Covered Entity, and in the time and manner reasonably designated by Covered Entity, to PHI in a Designated Record Set, to Covered Entity or, as directed by Covered Entity, to an Individual in order to meet the individual identified as being entitled to access and copy such PHI, within five (5) business days of receipt of such a request from Covered Entityrequirements under 45 CFR §164.524.

Business Associate shall, at the request and direction of Covered Entity, make PHI maintained by Business Associate in a Designated Record Set available to Covered Entity, or, as directed by Covered Entity, to the individual identified as being entitled to access and copy such PHI, within five (5) business days of receipt of such a request from Covered Entity.

Access to PHI. 2.6.1. Business Associate shall, at the request and direction of Covered Entity, will make PHI maintained by Business Associate or its agents or subcontractors in a Designated Record Set Records Sets available to Covered Entity, or, or as directed by Covered EntityEntity to an Individual, to the individual identified as being entitled to access for inspection and copy such PHI, copying within five ten (510) business days of receipt of such a request from by Covered EntityEntity to enable Covered Entity to fulfill its obligations under the Privacy Rule, including, but not limited to, 45 CFR § 164.524.
