Operating or Performing Physician definition

Operating or Performing Physician means a licensed physician who has primary responsibility for the surgery or procedure performed physically performs the out-patient procedure or who supervises the other medical professionals performing such procedures.
Operating or Performing Physician means a licensed medical doctor, osteopathic physician, dentist, podiatrist, chiropractor, or advanced registered nurse practitioner who has primary responsibility for the surgery or procedure performed.
