Operating or Performing Physician definition

Operating or Performing Physician means a licensed physician who has primary responsibility for the surgery or procedure performed physically performs the out-patient procedure or who supervises the other medical professionals performing such procedures.
Operating or Performing Physician means a licensed medical doctor, osteopathic physician, dentist, podiatrist, chiropractor, or advanced registered nurse practitioner who has primary responsibility for the surgery or procedure performed. The operating or performing physician may be the attending physician.
