Travel Immunizations. This agreement covers additional immunizations only when rendered before travel. Immunizations are only covered to the extent that such immunizations are recommended for adults and children by the Centers for Disease Control and Prevention (CDC). Contraceptive Methods and Sterilization Procedures for Women This agreement provides coverage for the following:  FDA approved contraceptive drugs requiring a prescription;  FDA approved contraceptive devices requiring a prescription;  sterilization services for women with reproductive capacity;  barrier method (cervical cap or diaphragm) fitted and supplied during an office visit; and  surgical services, including but not limited to tubal ligation and insertion/removal of IUD.
