Common use of Transition of Covered Persons Clause in Contracts

Transition of Covered Persons. Provider shall cooperate with Subcontractor and Health Plan in the event an immediate transfer to another primary care physician or Medicaid managed care contractor is warranted if the Covered Person’s health or safety is in jeopardy, as may be required under law.
