Training Report a. The training report includes documentation of training that covers the areas specified in Section I.
b. Documentation shall include:
i. Names of PATH staff responsible for clinical practice and other PATH services training activities;
ii. Staff participating in training within the past twelve months; and
iii. Name, title, and credentials of the trainer.
c. Grantee shall submit documentation of training as requested by System Agency.
Training Report. Within one hundred and twenty (120) days after the receipt of HHS’ approval of the Training Program required by section V.G, the FMCNA Covered Entities shall submit a written report to HHS known as the “Training Report,” which shall consist of:
1. A copy of all training materials used for the training required by this CAP, a description of the training including a summary of the topics covered, the length of the session(s) and a schedule of when the training session(s) were held;
2. An attestation signed by an officer or director of FMCNA that all applicable members of the workforce of the FMCNA Covered Entities have completed the initial training required by section V.G and have executed the training certifications required by paragraph V.G.4; and
3. An attestation signed by an officer or director of FMCNA that he/she has reviewed the Training Report, made a reasonable inquiry regarding its content and believes that, upon such inquiry, the information is accurate and truthful.
Training Report. Attachment B, Budget and Targets, is hereby amended and restated in its entirety with
Training Report. CONTRACTOR shall submit to ADMINISTRATOR, within thirty (30) 35 calendar days of the event, a report of each training or conference attended by any staff member(s), and 36 paid in part or in whole through this Agreement. The report shall be no more than two (2) pages in 37 length and shall include the training title, purpose, host organization (e.g., Center for Applied Research 1 Solutions, Inc.), a list of key materials and handouts, a summary of what was learned, and an analysis of 2 potential application to alcohol and other drug prevention services provided pursuant to this Agreement. 3 When multiple staff members attend the same training or conference, a single collaborative report may 4 be submitted. After submission, training reports may be distributed to other contracted providers at the 5 discretion of ADMINISTRATOR.
Training Report a. No later than October 16, 2019, Grantee shall send by email to System Agency documentation of training that covers the areas specified in Section 2.4 K.
b. Grantee shall submit documentation of training as requested by System Agency.

The report must include the names and titles of the individuals who attended the training, the date of each training session, the agenda, copies of the training materials or presentations, and the names and qualifications of the trainers.

Training Report. As part of the Budget Period 3 annual progress report due September 30, 2015, awardees must report on funded trainings conducted during Budget Period 3. The template for this report can be found in the PERFORMS Resource Library. Awardees are required to use this form for application planning and throughout the budget period to track trainings. The completed template must be submitted by September 30, 2015.
