Therapy Program Services. a) The Provider shall provide Medical Assessments: an examination or records consultation resulting in a report of the nature, severity and prognosis of physical trauma sustained by the child victim performed by a primary care or emergency care physician, a licensed physician’s assistant, or advanced registered nurse practitioner. Medical Assessments are subject to the provisions of FS 960.28(1).
